GRANT FINAL REPORT

The Northwest Minnesota Foundation and the Thief River Falls Area Community Fund require a final report upon completion of each project for which a grant is awarded.  Please provide the following information.  Feel free to attach additional information or other materials that further reflect the outcome of your project.

THE FINAL REPORT MUST BE SUBMITTED BEFORE ANY ADDITIONAL GRANTS WILL BE CONSIDERED OR AWARDED TO YOUR ORGANIZATION.
